We’re working with local partners to ensure that Disabled children and their families can access the support, information, and opportunities they need to thrive.
We believe that every family should have the right to choice, control, and inclusion – and we’re building this new service to remove barriers and make that a reality.

This new service is currently being planned and designed in partnership with local organisations, families, and professionals.
Our goal is to create a service that reflects the needs and priorities of Disabled children and their families, offering the right support at the right time.
We intend to invest in this area to strengthen local provision and build a more connected support network across the borough.
We will work in close collaboration with Parentsactive, the local forum for parents and carers of Disabled children and young people in Hammersmith & Fulham.
The voices of Disabled children and young people will be central to everything we do, ensuring they help shape and influence how the service develops.
